A web system that took over the paper archive of a social welfare home. It supports the full path of resident documentation - from application and admission, through an individual support plan, to day-to-day care. Deployed for Social Welfare Homes subordinate to the capital city of Warsaw.

Complete resident e-documentation
The system gathers the full documentation of every resident in one place - from the application and admission decision, through the support plan, to current care. No more paper archives: the data is always up to date, available and fully traceable.
Planning
ISP - Individual Support Plan
It supports creating Individual Support Plans based on a catalogue of dozens of services and procedures. The system helps arrange the plan, select services and assign responsible employees - replacing laborious paper procedures with a predictable digital process.
Communication
Notifications and reminders
The team communicates inside the system: a notification reaches the whole facility, a selected group or the employees responsible for a specific service. The system itself reminds about upcoming deadlines - e.g. an expiring ID card or an administrative decision.
Availability
Intuitive navigation and action search
We designed the menu in direct cooperation with social welfare home employees to make the system natural to use. The built-in search with auto-completion reaches any module within seconds - even in a system of hundreds of forms.
Data
Data exports and reports
An advanced print wizard for registers, lists and records: you select the needed data from a table and export it to PDF, XLSX or CSV. Reports that used to take hours are created in minutes.
Documents
Printouts and forms
A generator of decisions, applications and forms: the system automatically fills in the resident’s data and you only indicate the recipient and print. PDF and DOC files - ready for circulation between institutions.
Security
GDPR compliance
Access to personal data is controlled and fully logged: the system records every profile view and every change of data. Auditability as a standard - without extra tools.
